Waverley is commited to helping it's patients utilize the maximum allowable benefits. Before surgery, please familiarize yourself with your insurance plan, bring a copy of your insurance identification card with you. Following the verification of insurance benefits with your carrier, Waverley will notify you regarding any deductible or co-insurance payments.

Workers' Compensation

Waverley accepts Workers' Compensation and will rely on the physician's office to obtain the Workers' Compensation authorization as evidence of coverage.

Payment for Services

Payment for services is due on the day of surgery. We accept cash, cashiers checks, personal checks and credit cards. All deductibles and co-pays must be paid on the day of service.

Waverley recognizes its obligation to remain flexible in financial matters. In the event that you cannot meet any of the above financial requirements, we will work with you to make other financial payments. However, you must contact us in advance of your surgery if you wish to make alternative financial arrangements.

If you have questions or concerns about insurance or billing information, please contact Waverley Business Office at (408) 879-1820. The business office hours are from 9:00am to 5:00pm.

Be advised that you will also receive bills from your anesthesiologist, physician, or any other diagnostic services if needed from a pathologist, laboratory, radiologist or other services.
